Person-Centred Care
At The Bess and Moe Greenberg Family Hillel Lodge Long-Term Care Home of Ottawa (Hillel Lodge), we are dedicated to advancing our vision of leading the way in person-centred care. Under the person-centred care model, each resident’s values, preferences, interests, needs, and goals guide every aspect of their care and the services we provide. Person-centred care aims to support residents by managing chronic and complex health conditions while enabling them to live every day to the fullest.
Hillel Lodge is committed to ensuring that each of our 121 residents have their needs met in a person-centred environment. We achieve this by:
- Treating Residents with Dignity: We recognize each resident as a unique individual with distinct needs, treating them with compassion and respect.
- Empowering Decision-Making: We support residents in making informed decisions about their care.
- Offering Choices: We provide a variety of options for care and services to meet the diverse needs of our residents.
- Respecting Religious and Cultural Needs: We ensure that our care aligns with each resident's religious and cultural preferences.
- Adopting a Holistic Approach: We focus on addressing the comprehensive needs of our residents through integrated services.
Striving for Excellence
To uphold these principles of our Person-Centred Care model, Hillel Lodge will:
- Continuously Assess Resident Needs: We will strive to understand and adapt to the evolving needs of our residents.
- Engage Leadership and Team Members: Our leadership and all team members will embrace this philosophy in their daily work.
- Review Programs and Services: We will regularly evaluate our programs and service delivery to ensure they align with our person-centred philosophy.
Families as Partners in Care
Hillel Lodge strives to provide services designed to not only meet, but exceed, the expectations of residents and those close to them. Foundational to this care is our mission, vision, and values, which place the resident as the primary focus of our person-centred care philosophy. We strongly believe residents’ families are important members of the team and invite them to take an active role in the care planning process by sharing their knowledge of the preferences and life history of their loved one so that together we can plan for the optimal delivery of care.
