Social Work

The Social Services Department plays a vital role in enhancing the quality of life for residents at Hillel Lodge, helping to create a warm and homelike atmosphere. The dedicated social work team provides:

Support Services: Offering supportive listening to residents and their families.

Admissions and Discharges: Facilitating admission, placement, and discharge processes.

Social Work & Advocacy: Representing residents' needs and acting as liaisons with Family and Resident Councils.

Resident Care Conferences

Within six weeks of admission, the resident and family are invited to meet with the multidisciplinary team to discuss the initial care plan. The core staff members on the team consist of the physician, neighbourhood registered nurse (RN) / registered practical nurses (RPN), a member of the social work team with input from the registered dietitian, therapeutics, and recreation teams. Residents and family members are invited to the initial care conference and the subsequent annual care conferences to provide input on the best ways to provide care and support the resident.

Availability and Services

The Social Work Department is available during business hours, Monday through Friday. The Resident & Family Advocate facilitates initial and annual resident care conferences, organizes ad hoc family meetings, and provides caregiver support. She is always ready to assist when needed.

The team regularly collaborates with residents to assess changes in cognition and circumstances, ensuring that additional care is available as necessary. They also offer services for transition, supportive counseling, and end-of-life planning.

Primary Contacts

Julie Mitchell, the Resident and Family Advocate, typically serves as the first point of contact and remains available throughout a resident's stay. She works under the guidance of Lisa Rossman, Director of Resident Experience, who is also available to support residents and families.
